Form Cms-588 Electronic Funds Transfer (Eft) Authorization Agreement

The Centers for Medicare & Medicaid Services (CMS) requires all providers to submit a CMS-588 Electronic Funds (EFT) Authorization Agreement Form to receive payments electronically. Once suppliers are registered with the LFS, the EFT agreement cannot be terminated and suppliers cannot request paper cheques. Suppliers can register with the LFS and, if necessary, update or change the bank details using the CMS-588 form. Suppliers can also submit the form electronically via a web application via PECOS.
